Community Care Home Inc. (CCHI) is a Community Residential Alternative (CRA) provider licensed in the state of Georgia, which receives funding through NOW/Comp waivers program.
We serve individuals with intellectual and developmental disabilities who require long term supports in a residential setting where it would most benefit their personal development.
We serve clients diagnosed with an Intellectual Disability, Autism, Failure to Thrive, Downs Syndrome, Cerebral Palsy, PICA, Spina Bifida, Prader Willi Syndrome for example.
There is no cost to admit someone to CCHI, however they must meet the criteria to be enrolled in the state’s Medicaid program.
The eligibility criteria for obtaining services includes:
- Initial diagnosis of an intellectual disability prior to the age of eighteen (18).
- IQ of 70 or less, as determined by a recent standardized intelligence exam.
Placement is determined on a case by case basis however we consider age, individual supports, social skills, medical needs, etc. in locating the best fit.
